Adventure Spirit Forms
For all of our rock, ice, and alpine adventures, please link to the following documents, fill them out completely, and sign. Then either scan and email the forms or mail them to Adventure Spirit, 120 Lakeview Terrace #7, Burlington, VT 05401. Please send these in advance of your adventure. If you have any questions or concerns regarding your health and fitness and the adventure in which you plan to participate, please let us know.
